They've been getting better at making them unnoticeable or the info printed right into the inside of the shirt. So those I leave since I don't even feel them. But others that feel like claws scratching me, then they get ripped right out

I remember doing this often in the past, but very rarely nowadays. E.g. in the shirt I'm wearing now, the tag is made of some soft synthetic material that I hadn't thus far paid attention to. Maybe coarse, scratchy tags have been more common in men's clothes but now aren't??

 

If I notice it while wearing it, though, it has to go.

No. I attempt to rip them out first. If there's too much resistance I stop as I don't want to damage the clothes and instead I grab a need and slowly dethread the tag out.
